Mobile Dimension Fairleads

Hi folks!
I have a job that needs to use the block type set up, but I don't have all the necessary parts. I'm looking for a set of fairleads for a Mobile Dimension 128 pre hydro-static (I don't think that matters). Part# in my manual is:5145-FA for the front and 5145-RA for the rear, I think they are the same.

I have the the shoes, but not the fairleads and hooks and Mobile dimension is out of stock indefinitely and doesn't have any info on them. So if any body has any they are willing to sell me let me know. Or of some one could post some good photos with dimensions of one I might be able to fab some up.
